Our RN GCM's complete a comprehensive assessment in the home, hospital, or current living setting to identify needs, eligibility for assistance, and recommend a Plan of Care.
Finding "A New Home"
- Recommend kind and level of care or help needed
- Perform on-site evaluations of nursing home and assisted living settings
- Provide information regarding quality, cost, and availability of any setting
- Diminish senior's resistance to a move
- Assist with adjustment to a new home
Arranging and Overseeing Home Care
- Advise on quality, cost, and kinds of help needed and offered
- Screen and arrange hourly and live-in help
- Help senior arrange home maintenance
Case Management
- Manage care for the senior with out-of-town family
- Support local caregiver(s) in making difficult decisions
- Conduct on-going assessments, monitor, and implement needed changes in care
- Assist with the set-up and monitoring of medications
- Advise senior/family of significant health changes and make knowledgeable recommendations
- Coordinate and escort to medical appointments, tests and surgeries
- Encourage the senior in promotion of health and prevention of illness or accidents
- Collaborate with physicians and other health care professionals
- Advocate for senior with service provider, nursing home, assisted living setting, or hospital

For clients who would benefit from companionship and conversation, either at home, in the hospital, rehabilitation center, or new living setting when work, travel, health, or busy schedules don't allow you to visit. A Family Friend works in collaboration with your Professional Geriatric Care Manager to provide quality services at an affordable cost. |
What can a client expect from A Family Friend?
- Companionship visits to facilitate adjustment to new living settings
- Escort to and participate with the client in a scheduled activity
- Facilitate introductions to other residents with similar interests
- Share conversation, memories and interests
- Organize photo albums and Memory Books with their stories
- Read aloud, write notes and help with personal correspondence
- Initiate handcrafts, play games or cards
- Celebrate holidays and birthdays
- Visits several times a month
- Possible help with simple errands or purchases for personal needs
- Convey client's needs, preferences, or wishes to staff and family
- Provide the visible presence of an interested, involved advocate
- Follow-up telephone reports to family
