Company History and Philosophy

Diane Hischke, R.N., M.S.N., C.M.C., a Clinical Nurse Specialist, founded Serving Seniors, Inc. in 1992. She continues to serve as President and a direct provider of geriatric care management services to seniors and their families.

Diane's personal experience of caring for her mother who had Alzheimer's Disease was instrumental in her recognition of the need for such professional help for seniors and their caregivers. Her experience guides her in the development of the practice and her commitment to personalized, quality care.

Diane Hischke believes that Registered Nurses who specialize in gerontology or gero-psychiatric nursing, and have extensive experience working in the community, are the most effective geriatric care managers. All professional care managers in the practice:

Are licensed by the State of Michigan Board of Nursing as Registered Nurses
May also have social work degrees and Graduate Certificates in Gerontology
Share a total of nearly 80 years of professional work experience

The Nurse Geriatric Care Managers:

Assess and screen for mental and physical health conditions in the home or hospital
Are knowledgable of the differences between normal aging and symptoms of illness
Make well researched referrals to other health care professionals as needed
Collaborate as a member of the senior's health care team
Strongly advocate for their clients when needed
Do home visits and advocate for their clients in all living and treatment settings, including nursing homes and hospitals
Recognize the value of working with the entire family
Counsel and support the family, as well as the senior
Research services and resources in the community for quality, cost, and availability
Diminish senior's resistance to a move
Encourage the senior in promotion of health and prevention of illness or accidents
Screen and arrange hourly and live-in help
Help senior arrange home maintenance
Are available after hours and on weekends when needed
